DIRECTOR OF NURSING

JOB DESCRIPTION

Provides overall planning, direction, coordination, management and supervision of all nursing and professional services in conjunction with the Administrator. Assures quality care by reviewing and assessing each service.

"Director of Nursing" means a registered nurse who is a direct employee, of the agency and who is a graduate of an approved school of nursing and is licensed in this state; who has at least year of home health experience and at least 1 year of supervisory experience as a registered nurse; and who is responsible for overseeing the professional nursing and home health aid delivery of services of the agency.

Lines of authority and reporting responsibilities: Report to the Administrator

DUTIES AND RESPONSIBILITIES:

(a) The Director of Nursing of the Home Health Agency shall:

l. Meet the criteria as defined in s. 400.462(10), F.S. (See above)

2. Supervise or manage, directly or through qualified subordinates, all personnel who provide direct patient care.

3. Ensure that the professional standards of community nursing practices are maintained by all nurses providing care. Ensure HIPAA guidelines and procedures are maintained.

4. Maintain and adhere to agency procedure and patient care policy manuals.

5. Designate in writing a direct employee, who meets the qualifications, to serve as on-site alternate director of nursing during

absences of the DON. This person will be available during designated business hours, when the DON is not available. Available during designated business hours means being readily available on the premises or by telecommunications. During such absences, the on-site alternate DON will have the responsibility and authority for the daily clinical operation of the Home Health Agency.

(b) If the Administrator is not a Physician or Registered Nurse, the DON shall:

1. Establish service policies and procedures in compliance with state health statutes and administrative rules, which generally conform to recommended Centers for Disease Control (CDC) and Occupational Safety and Health agency (OSHA) guidelines for safety, universal precautions and infection control procedures.

2. Employ and evaluate nursing personnel.

3. Coordinate patient care services.

Set and adopts policies for, and keep records of criteria for admission to service, case assignments and case management.

(c) The DON act as Performance Improvement Coordinator, shall establish and conduct an ongoing Quality Assurance Program which assures:

Case assignment and management is appropriate, adequate, and consistent with the plan of care, medical regimen and patient needs.

Nursing and other services provided to the patient are coordinated, appropriate, adequate, and consistent with plans of care.

All services and outcomes are completely and legibly documented, dated and signed in the clinical service record.

Confidentiality of patient data is maintained.

Findings (outcomes) of the QA program are used to improve services.

In an agency with less than a total of ten (10) full time equivalent employees contracted personnel, the DON may also be the Administrator .

If an individual serves as the DON of more than one licensed agency, Pursuant to s.400.462 (10), F.S., a designated alternate DON must be available during designated business hours, at each additional agency, which has the responsibility and authority for the clinical operation. Available during designated business hours means being readily available on the premises or by telecommunications.

DIRECTOR OF NURSING OVERSIGHT

The DON shall establish process to verify that skilled nursing and personal care services were provided.

The director of nursing shall establish a process to verify that skilled nursing and personal care services were provided. When requested by an AHCA employee, the director of nursing shall provide a certified report that lists the home health services provided by a specified direct service staff person or contracted staff person for a specified time period. A certified report shall be in the form of a written or typed document or computer printout and signed by the director of nursing. The report must be provided to the surveyor within two hours of the request, unless the time period requested is longer than one year, then the report must be provided within three hours of the request.

The director of nursing shall establish, and conduct an ongoing quality assurance program. The program shall include at least quarterly, documentation of the review of the care and services of a sample of both active and closed clinical records by the director of nursing or his or her delegate. The director of nursing assumes overall responsibility for the quality assurance program.

PHYSICAL REQUIREMENTS:

This Job Classification will have a Potential risk for Occupational Exposure to Blood and other Potential Infections body fluids, protective equipment will be provided by our Agency to limit the exposure and will promote self protection practices in the delivery of the Home Health Care, to provide appropriate treatment to home health care workers in the event of exposure incident and to promote compliance with the universal precautions.

Completes educational requirements as per state and regulatory agency standards.

THE ADMINISTRATOR:

JOB SUMMARY:

The administrator is a qualified direct employee appointed by the Agency to administer, who

direct and coordinate all administrative and financial activities of the Agency. "Administrator"

means a direct employee, the administrator must be a licensed physician, or registered nurse licensed to practice in this state, and holds an undergraduate degree, having at least 1year of supervisory or administrative experience in home health care or related healthcare program.

1. Organizes and directs the Agency's ongoing functions:

Planning, organizing, directing and evaluating the organization's ongoing operations to ensure the provision of adequate and appropriate care and services

(a) Superintends and be responsible for the total operation of the Agency. Act as Compliance Officer.

(b) Plans overall development and administration of the Home Health Services as act forth in the Conditions of Participation for Home Health Agencies under the direction of the Board of Directors (Governing Body), with the advice of our Professional staff.

(c) Meets with Board of Director (owners), Director of Nursing, Clinical Manager and Case Manager, to solve administrative problems.

(d) Oversees the development of standards and methods to measure Agency Activities.

(e) Participates in the review, analysis and appraisal of the effectiveness of the total Agency program.

(f) Provides for continuing evaluation of the program by:

(1) Evaluating service policies and functions, and recommending proposals for changes or study of problems which affect the Agency.

(2) Evaluating of the individuals in the program in relation to established standards and the individuals professional development.

(g) Employs qualified personnel and ensures the orientation of new staff and opportunities for continuing education experience for the staff.

(h) In addition, the Administrator shall:

(1) Be knowledgeable with the rules/statute of the State and ensure compliance with them

by the Agency.

(2) Be responsible for informing the employees with the law, rules, and home health

statutes of the State and shall have copies of the rules available for their use.

(3) Be responsible for the completion, keeping and submission of such reports and records

a s required by the State.

(4) Designate in writing a similarly qualified alternate direct employee to be his authorized

representative at all times during operating hours in the absence of the administrator, which provides the agency with full control over all operational duties and responsibilities to serve as on-site alternate administrator. This person will be available during designate business hours, when the administrator is not available. Available during designated business hours means

being readily available on the premises or by telecommunications. During the absence of the administrator, the on-site alternate administrator will have the responsibility and authority for the daily operation of the agency.

(5) Maintain a current organizational chart to show lines of authority to the patient level.

(6) Clearly identify and make public services provided by the Agency and the geographic area in which these services are available.

(7) Maintain an office facility for the Agency which is large enough for efficient staff work, adequately equipped, and which provides for a safe working environment, meeting local ordinances and fire regulations

2. Maintains ongoing liaison among the governing body, agency committees and the staff. Home Health Agency Job Descriptions

Maintain/create HIPAA Compliance Manual, and reenforce the compliance. Implementing Governing Body directives, participate in QAPI program &organizational P&P's. Ensure that appropriate service policies are developed and implemented.

(a) Submits progress reports to the Board of Directors and Agency's Committees.

(1) Notifies the Board of Directors within 72 hours of notice of any suit or threatened legal action against the Agency.

(b) Supervises preparation of periodic reports to the Governing Body concerning various phases of Agency operation and its financial conditions.

(c) Reports decisions and recommendations from the Board of Directors and Agency's committees to the professional staff.

(d) Meets regularly with the agency committees and other professional supervisory group for mutual sharing and problem solving.

3. Complying with applicable laws &regulations (Local, State, Federal, and accreditation standards if applicable).

(a) Develops cooperative relationships with other Agencies for exchange of information and Services, and with community agencies to develop understanding of Agency program.

(b) Contract local, state and national associations and participates in meetings and conventions.

4. Recruiting, employing and retaining qualified personnel to maintain appropriate staffing levels. Implements an effective budgeting and accounting system.

5. Ensuring adequate staff education. Ensuring staff development including orientation, in-service education, continuing education and evaluation of staff. Direct and monitor organizational performance improvement activities; and assuring that skilled nursing and other therapeutic services furnished are under the supervision and direction of a physician or a registered nurse. 6.Establishing and maintaining effective channels of communication. Completing performance evaluations on subordinate staff in accordance with organizational policy

.7 Ensuring program personnel have current clinical information and current practices. Directing/monitoring organizational Performance Improvement activities, QAPI program.

8. Managing operations in accordance with established fiscal planning, budgeting and management of operations in accordance with established fiscal parameters.

9. Planning, developing, implementing, administering &evaluating programs

10. Assuring that skilled nursing and other therapeutic services furnished are under the

supervision and direction of the physician or a registered nurse. Representing the organization to other group, organizations &to other general public.

11. Assuring appropriate staff supervision during all operating hours.

12. Assuring the development and qualifications for professional services and the assignment of

personnel. Informing the GB & staff of current organizational, community & industry trends 13. Ensuring the accuracy of public information materials and activities

14. Informing the governing body, staff and agency's committees of current organizational, community, and industry trends.

QUALIFICATIONS:

Must be a licensed Physician, or Registered Nurse, or holds an undergraduate degree, and

has at least 1year of supervisory or administrative experience in home health care or related

health programs. Be knowledgeable of the federal conditions of participation (CoPs), statute and administrative rules of the State, and ensure compliance with them by the Agency. Must show exceptional leadership ability and good professional judgement. Maintains administrative and/or supervisory responsibilities when care is provided under contract.

If our Agency changes the administrator or alternate administrator, we shall notify the State, Home Care unit office prior to or on the date of the change. Notification shall consist of submission

of the person's name, professional resume, professional license, fi applicable, and a copy of the Affidavit of Compliance with Criminal Background Check . The administrator also must submit level 2 screening, or inform the Home Care Unit that level 2 screening was previously submitted
