In this role you will: The Family Connection Specialist (FCS)will be able to provide telephonic and in-person assistance in connecting individuals/families to mental health and community-based services.

The FCS will administer the Child Center Health and Wellness Questionnaire (CC-HWQ) to screen for unaddressed needs related to the social determinants of health. Based on the results, staff will work with the client/family to provide information, make referrals and advocate for services, as needed. The CC-HWQ uncovers unaddressed issues negatively impacting health and wellbeing, such as behavioral or physical health problems, financial instability, food insecurity, or unsafe housing. Essential Functions: •The FCS will perform constant and regular outreach so they can recruit clients, and understand the communities’ needs.

  • The FCS will provide referrals as needed to a wide variety of services, from mental health to vocational assistance and anything else. •They will also help clients connect to The Child Center’s Benefits Access program (in-person or virtually), which provides free counseling on entitlements, health insurance, financial guidance, social services, and legal services. •The Family Connection Specialist will also provide advocacy services; •Will provide support and educate on how to be a self-advocate. •In order to pinpoint the exact areas to target for this program, the FCS will conduct community based research to determine the determine the characteristics of the community’s youth, and hold various listening tours and focus groups with residents of the community and our community messengers), such as those who provide vital services (e.g.
barbershops, beauty parlors, and other gathering sites) to learn what populations have been particularly impacted by racism, racial inequity and related racial disparities. •At the same time, the FCS will work closely with The Child Center’s Communications Department to create linguistically and ethnically current culturally competent, culturally sensitive ways of reaching the community, multi-lingual flyers and other collateral materials for distribution to community-based organizations, community centers, NYCHA housing, religious institutions and hospitals in the Bronx and Brooklyn. Supervisory Responsibilities: None Minimum Qualifications: Master’s Degree in Psychology, in Counselling or Social Work from an accredited School. Bilingual in one or more of the following: Spanish, Mandarin, Cantonese, Bengali and others, may be required depending on client needs.

Proficiency in Microsoft Office. Ability to plan and carry out assignments independently. Ability to prioritize, adhere to timelines and multi-task. Qualifications Preferred: LMSW, LMHC, LMFT, LCSW certification.
