Job Summary:
Klarus Hospice is looking for an Intake Coordinator/Medical Records Specialist to join our San Antonio team. The primary function of the Intake Coordinator is to facilitate the referral intake process, coordinate care with the interdisciplinary group, the patient, their family, and the referral source.
Requirements:Job Qualifications:
Education: Graduate of an accredited school of Vocational/Practical nursing.
Licensure: Current state license as a Licensed Vocational/Practical Nurse or a multi-state license issued by a state member of the National Nurse Licensure Compact (NLC). Current drivers license.
Experience: One year experience as a Licensed Vocational/Practical Nurse, hospice experience preferred.
Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
Transportation: Reliable transportation. Valid and current auto liability insurance.
Environmental and Working Conditions:
Works in a routine office environment; noise level may be moderately high; must have the ability to work a flexible schedule and extended hours. Possible exposure to toxic materials, toner, etc.
Physical and Mental Effort:
Requires working under some stressful conditions to meet deadlines and patient/client needs, and to make quick decisions. Requires excellent problem solving skills.
